Welp, West Virginia is closed now, too. There go my camping plans. I was really looking forward to the break, too. Ever since moo goo gai panic hit, work has been crazy and I haven’t had a full day off in three weeks. At least I took a walk and finally saw some sunlight for a while. Hope everyone else is holding up well; I’m starting to get a bit of cabin fever, but people have lost their fucking minds and started photographing and shaming people on fb for daring to walk around the park. Fuck those people.
Finally had some decent weather for a break, and I managed to sneak out to get a 10 mile bike ride in. By the time the ride was coming to an end, you could feel the temperature dropping already. Thunderstorms predicted all weekend (but in the 60’s), and back into the 40’s next week.
I’m still hoping for the Europe trip in the summer, but that’s looking like a slim chance at this point.

I’ve never seen such a level of panic across the country and it’s been honestly fucking with my head and making me wonder if someone slipped drugs in the water supply or something. I just want the country to take a giant chill pill and start approaching this with an eye toward reasonable risk management measures.
It’s novel because it’s a previously unseen coronavirus. It’s in the same species as the SARS coronavirus. While there are a large number of coronaviruses, most do not affect humans because we lack the appropriate cellular receptors for the virus to attach. Unless viruses can attach to and enter cells, they have no effect on an organism. Most coronaviridae are found in bat reservoirs. Unfortunately because this virus is different, we have no immunity to it or real ability to resist it.
SARS and Covid-19 both cause severe inflammation of the respiratory tract. The fear is that we will have a similar situation to what happened when European explorers brought new diseases to the Americas. The population died by the millions. That’s why epidemiologists are frightened. Sure, you can argue that they may be going overboard, but it’s not out of the realm of possibility.
Influenza is not a coronavirus, it is a orthomyxovirus. It’s a very different thing. Yes, it’s also a virus, but it’s also a virus in the same way that a mouse and a elephant are both mammals.
So no, this isn’t the flu. This is a new disease and we don’t have any good treatments for it.
But do you think that shutting down the entire country is a reasonable or effective response that prevents more death and misery than it causes? I’m of the opinion that isolating vulnerable populations and encouraging mask wearing in public, plus stronger hygiene measures, etc, would be effective without causing nearly the economic damage we’re taking on. I think we went overboard, and caused a lot more harm than good on early and incomplete advice.
I think you’re probably right and am very concerned that we are flushing all of our civil liberties and protections down the toilet because we are frightened, but there are actual grounds for the fear. The virus is not imaginary.
I’ve treated several patients who were seriously ill with it so far though none (thankfully have died). These were previously healthy people who became extremely ill within a few hours and had to come into the hospital for respiratory support.
I’d contend we could have done a much better job, but the reality is no politician wants to be accused of “not doing enough”. And let’s face it, the news isn’t around to tell us it’s all going to be ok.

I think we are right to be concerned and to take strong public health measures to retard the spread, but I also think we can do that without destroying civil liberties. As a selfish example – my camping plans involved staying in my travel trailer with its own bathroom and kitchen on a site with its own firepit. The only exposure risk we faced was when my kids went to the playground and I could make them wash their hands and wear masks. Instead, everyone has to stay home and the state park is out a week’s prepaid reservation. That doesn’t make anyone happier or healthier or richer.

Several viruses cause the common cold.
Yes and no. The common cold is really a catch all term for a viral upper respiratory infection, with symptoms generally including some mixture of fever, cough, nasal/sinus congestion, body aches, and sore throat. The most common cause are species of rhinovirus but there are a lot of viruses that can cause it including influenza viruses and coronaviruses. (among a bunch of others). The current coronavirus probably could cause a common cold illness, but in some people for reasons we don’t really understand it causes severe lower respiratory (lung) symptoms and systemic inflammatory cascades that can be rapidly fatal.
So, what I’ve been hearing about the virus only being fatal in mostly elderly or already health compromised people is not true? It just has severe symptoms in some people for reasons we don’t understand?
No it hits the elderly especially hard for a number of reasons, not the least of which is that old people have decreased lung function (and function in general) relative to younger people. They are also far more likely to have underlying diseases that predispose them to death from any severe infection, such as heart or kidney problems, COPD, and the like.
Younger people, with strong hearts, lungs, and so forth, are more able to weather the storm of the massive inflammatory response that covid-19 can trigger (again, we don’t know for sure why some people have symptoms like a really bad cold and others go into respiratory failure). The older you are, the less reserve you are likely to have and the higher your risk of death.
No, that’s the question I’m asking. Are you saying that some people, assuming they are healthy and younger, have a far more severe response, for unknown reasons? I have not heard that before.
I would assume that if that is true, we’d better be finding out why. There must be some sort of clue by now with as many cases as we’ve had?
Hopefully we can figure that out but I haven’t seen anyone who seems to have any idea.
I took care of a previously healthy lady in her 50s recently who had been fine with normal vital signs the day before I saw her. When she got to me she was pale, sweating, wheezing, and had infiltrates throughout her lungs. She got admitted to the hospital and got better. She went home after three days.
One of my partners treated a likewise previously healthy man in his 50s who is still in the ICU on a ventilator after over a week of the same therapy. What’s the difference? Why do some people respond to treatments that have no effect on others? We aren’t very close to answering that question in most areas. It’s not an easy thing to identify.
Ok, we know it is horrible for those of us it is going to be horrible for. What are the odds I am going to acquire this ailment? How long has it been here and how many times have I came into contact with it? You obviously know much about these sort of things. Why are we know shutting businesses down because of this horrible thing when we decade after decade ride out the multitude of all the previous horrible things? Is this the most horribelestest of things?
From what we are seeing so far, about 80% of those that contract covid-19 that become symptomatic will have a milder URI (cold) type illness. An unknown number of us can show evidence of infection but never have significant symptoms. About 15% will have severe enough illness to need to be in the hospital. About 5% will have a critical illness requiring ICU care. The last are the patients that will have a high likelihood of death even with the best care we can give.
As to your odds of actually contracting the disease, the short answer is I don’t know and I don’t think anyone else does either. Coronaviruses are highly infectious and can live outside the body for a fair amount of time. The upshot is that it’s probably very likely if you are in close proximity to an infected individual for a prolonged period of time. Passing them in the hallway? Probably less so, but the risk isn’t zero.
We can’t really study how infectious a disease is easily because that would require knowingly exposing people to a potentially deadly disease for research purposes. That sort of thing is frowned upon (and it should be).
I’m an emergency medicine physician, so unfortunately I’m up to my ass in this stuff right now. I try to know as much as I can about it, but there’s so little in the way of reliable information and the guidelines seem to have changed every damn shift.

I’ll reminisce back to my musing from a few days ago.
When I was a kid, we had such a rich diversity of horrific plagues that were sure to visit you. Mumps, Measles, Chickenpox. You WERE going to get all of these, along with influenza of every sort, and pneumonia. Smallpox and Polio were things that had only recently been mostly defeated.
The way I see it. We’ve just made being human so safe in the past few decades, I mean except for the accepted Western diseases, you know, heart disease and diabetes, from our shitty fucking lifestyle, that people freak the fuck out now over anything when it does happen. This isn’t the damn Bubonic Plague or Ebola, or even the Spanish Flu. It’s just that we’re so goddamn safe anymore that we freak the fuck out if something bad does happen.
I live in Nashville.
We don’t know if it’s the Spanish Flu yet. I think probably not, but we have no idea when or if the new cases will plateau and begin to diminish. According to the Chinese they have stopped having a significant number of new cases, but I wouldn’t trust them to count the number of driveways I have at my house (one).
Our dilemma is that we are rapidly running out of places to put these people in endemic areas as well as medicines and equipment to treat them. We’re also running out of masks, gowns, and things so that we don’t get sick. That’s the theory behind the lockdown and social distancing. It’ll give us enough time to free up places to put patients and make stuff to help them and protect ourselves.
I’m not saying the absurd levels of panic are warranted. They aren’t. People are going to die. That’s certain. But we aren’t anywhere close to the number of deaths in a typical flu season, yet. Our goal is to keep the deaths to the absolute minimum possible. The trouble is we don’t know what that number is yet.
I’ll just continue with my tirade.
When I was 6 years old, Chickenpox almost killed me. I had it so bad, I wound up with pneumonia along with it and I was in the hospital a long time. I think there were several days I had no recollection of as I was so sick and they thought I was not going to make it.
That being said, I’m not too scared of this virus, even though my wife and I are over 50, so obviously more vulnerable.
What I am scared of is the actions being taken that are wrecking our economy and lifestyle to a degree that we can never recover from. After I worked my ass off my entire life to live comfortably.

In Baltimore. Outlying counties have almost no cases at all. One of my clients is one of the best known medical research organizations on the planet, so I’m sort of up to date on what is going on. Although I’m not a virologist or epidemiologist or anything that would really give me any authority to discuss this in detail, I think I definitely understand the economic consequences of what is going to happen over the next few weeks or months.
I agree with you Hyperion, but don’t let despair win. I believe that we will recover. Once the infection curve begins to fall (and it will), eventually people will realize that life will go on and they’ll get back to living. We might even have an economic boom. With any luck, this will bring people together and focus on family, friends, and things that are really important. Maybe we’ll even tell the social justice hucksters to go pound sand.
Or maybe not. But I can dream, can’t I? Just don’t live in despair. Do what you can for yourself, your family, your friends, and your community. We can’t ask for any more than that from each other.

I actually hallucinated Darth Vader attacking the “brush your teeth” kids in PSAs I was so sick. I’ve seen the US survive many a epidemic and disaster without willingly shutting down the country and deliberately destroying people’s livelihoods, and wonder what makes this virus so different. Or is it our society that has changed so much? I encourage mask wearing and hand washing… and personally I’m loving the total shutdown of the schools because my son was having a really rough time just for touching someone on the shoulder, but I recognize that this is a massive disruption for almost everybody else and would love to see a more measured response that balanced public health with productivity and commerce.

A look at TN stats say you guys have 76 people hospitalized thanks to the CV. Looking from the outside I am not seeing emergency. Are people freaking out or are the top men?

Here our ER census has been down considerably as have most in my hospital’s network. The usual ailments aren’t coming in nearly as much as usual, so in that sense you’re absolutely right.
On the other hand, we’re now having to gown up in full droplet precautions for every patient with any sort of respiratory complaint, fever, or anything else that might be tangentially related to covid. That adds a lot of time to what should be routine patient encounters. We haven’t run out of masks yet, but we’re being rationed to one per day. Plastic isolation gowns are also in short supply. I bought a bunch of trash bags so I could make my own, just in case.
Technically after each patient encounter we should be taking off our PPE in an isolated changing room, including the masks, as anything in a droplet patient room is potentially contaminated. Guess what? We don’t have changing rooms (nor any plans to make any) so we get to take them off in the patient room or in the hallway (both of which are huge no-nos from an infection control standpoint). We’re also being told that it’s totally ok to wear the same plastic gown and mask to see the next patient as long as we put on new gloves. In other words, we’re being told to do things we know are not good practice. Our policies are also changing every day and nobody knows how bad it’s actually going to get. Doctors and nurses watch the news too. We’re being told they think a surge is going to hit in 1-2 weeks.
As a consequence, everybody is on edge, even though we haven’t been hit yet. I do have friends practicing in more endemic areas and they’re reporting that it’s a nightmare.
Newsflash, Doc, we’re all being told to do stuff that we know is not good practice. Some of us are OK with that and others (me) are wondering who the fuck thought all this was a good idea.
I appreciate the offer, Lemon, but it’s a nationwide problem and already receiving publicity. We’ve got people in the community knitting hats and masks for us, but as much as we appreciate it, we need N-95 masks or respirators which you can’t crochet together.
One of our local hospitals had a huge PPE stockpile in a warehouse nearby just in case of this kind of thing. It was all ready to deploy. But do you happen to remember that huge tornado that hit Nashville a couple of weeks ago? All those supplies were destroyed along with the warehouse.
For now, we still have masks, gowns, and gloves. But we don’t know how long that will last. We’re being told it’s all going to be fine, but “hospital leadership” in general is synonymous with “incompetent peddlers of bullshit” so I’ll believe it when I see it.

Yes, we’ve done that before, but the last time anything close to this scale was H1N1 back in 2009-2010. Or do you mean the trash bags? Nope, that’s a first, but as I said I don’t believe a word hospital administrators tell me.
Once, I got a really bad head rush and fell down in a bathroom stall. Cut my chin pretty bad. Girlfriend insisted I go to the ER. The doc asked me why I passed out. I told him I thought it was from pushing myself too hard, not sleeping or eating right, drinking too much coffee, etc. He said “Bullshit, we all do that everyday at this hospital and we’re not passing out!” Then he put about 4 stitches too many in my chin, missing the local anesthetic by two on each side and making the scar twice as bad. I thought, “yeah, I guess you guys are all wound up here.”
I’m sorry you had that experience. Believe it or not the bathroom is probably the most common place people do pass out. Still, it’s sad that you didn’t have a better time of it.
I do my very best to treat patients as well as I can. Yes, I can get unbelievably frustrated, but it’s still a very rewarding job, if sometimes a thankless one.
I remember a few years ago I had a patient who overdosed on some medications and was brought in pulseless. We worked on him and got him back. I admitted him to the ICU. A couple of days later I had a few minutes of downtime and walked down the hall to see him as I had heard he pulled through and had just had his breathing tube removed. When I came in he was sitting up and holding his girlfriend’s hand.
I introduced myself and talked with him for a few minutes. I was able to fill in some of the blanks as to what had happened to him and how he ended up in the ICU. I told him the breathing tube that had just been removed was put there by me as I tried to get his heart and lungs to work. He expressed how he now understood how precious his life was and how much hell he put his loved ones through. I encouraged him to get help with his drug addiction and said my goodbyes.
I had almost cleared the door when I heard his weak voice say “Doc?”. I turned back to look at him. “Thanks for saving me”, he said. I didn’t know how to respond other than “you’re welcome”. I turned and left.
Haven’t had a lot of moments like that in my career, but I’ve held onto them as tightly as I could.
I’m just sorry you apparently got someone who hasn’t.
That’s totally cool. I’ve been to ERs enough to have had a few good experiences too. You’re a good man to care what a rando on the internet thinks. I am glad to see it. That hospital went out of business not too long later. It was used to film “Scrubs” exclusively for about three years after it closed and then became a massive condo park. I hope the best for you and all the medical staff. It’s gotta suck to be among the few people who almost certainly will get coughed at by a carrier.
You know it’s funny. I’ve seen a few of the medical shows on TV over the years, though I’ll be damned if I want to watch a drama about my job when I get home. They’re mostly bullshit, but the one show that actually got the medicine fairly right was Scrubs. Probably the rest of it too. I’m not sure what that says.

I was doing infectious disease as part of my residency during the H1N1 epidemic. Part of the reason there wasn’t a panic was Tamiflu had come on the market and was being sold as if not a cure, at least a very effective treatment. Plus we’d seen flu for years and years. There still was a lot of public concern, but you’re right that it was nothing on the level of the insanity we’ve been seeing.
This reminds me a lot of 9/11 actually. People were scared to death of terrorists and some were afraid to go anywhere in public. Luckily, there weren’t any further attacks and people gradually got back to their lives. We still managed to do plenty of dumb shit even in that short time though (Patriot Act, TSA, Homeland Security, etc). We don’t have that luxury now. People are going to keep dying for a while so the panic will continue.
Hate to say it, but people always die. It just pisses me off when those deaths are used to transform our country. This is no 9/11. This is an orchestrated panic.
Have to disagree. This is nothing like 9/11. I worked next door to the WTC at the time and there was no fear going back there to work. There was some panic over what people felt might be the danger from the smoke but I don’t remember any fear of further attacks.

Will a spray down of bleach mixture do the job?

That story has so many layers. First, Trump said “I hope…” and everyone quite willfully misrepresented what he said as some sort of edict that he would be lifting all restrictions by Easter. Several entire segments on national news shows were devoted to explaining how terrible and irresponsible it is for Trump to be ordering an end to this by Easter.
Smash cut to Cuomo just hours later, with a sign next to him that says “a better way” saying the identical things. I mean, word for word, point for point identical. And he’s a heroic leader.
Bonus… PBS has said they will no longer carry Trump’s press conferences because of all of his lies. I think it was in response to the bit about chloroquine being promising and needing fast research.
Despite his mediocre at best performance at these press conferences, he’s up in the polls. I think it is entirely down to the colossally dishonest and unfair treatment the press is giving him. The insanely irresponsible questions they keep asking are not lost on the people, and they are backfiring. Again.
And to top it all off, the hero of the left has managed to outpace even Italy in the spread of the virus. After all of DeBlasio’s bluster about not closing schools down, etc, they’ve exploded with virus – NYC has 1/4 of the entire country, and NY State has almost half. They were not the first to be affected, but they are damn sure going to bring everyone else down if they don’t get cut off from everywhere else. With thousands of infected New Yorkers traveling the country spreading infection with many nucleation sites all at once, I’m afraid we are going to have exactly the explosion of cases that we are all fighting to avoid.

Joe was chief of police in my town, back when it was unincorporated, and under a different name. So….not my town.
I just had an hour long discussion with a fellow traveler who watched it this week. We both agreed that the only decent person was the one armed chick. She’s nuts, but I like her. They managed to get almost 7 hours out of that story, and, in a weird way, they almost need it. Yes- the injured-but-loyal assistant is probably the only diamond in that turd mine. The drug kingpin at least was upfront about his shit, and seems somewhat less threatening and almost normal, compared to the others. Even his security guard was professional and pleasant.

One of my fellow dispatchers came to us all the way from Chickasha county, where Joe was held during his trial, and where he got a lot of his roadkill. Even had some dealings with another safari place there…not exactly sure of that situation, but it wasn’t covered in the doc. She was aware of him, but had no real dealings with Joe. She watched all of maybe an hour and turned it off in disgust. Says she can’t stand to see them held up as any kind of example of the area residents.
